Membership Cancellation

1. If you would like to put your membership on hold instead of canceling, you can submit a Membership Hold Request using the hold request link. If you’re sure you’d like to cancel, just complete and submit the form below with at least 30 days cancellation notice. This will serve as your written cancellation notice as required by your membership agreement.  Failure to submit your request 30 days prior to your billing date, will result in automatic withdrawal of payment for that month. 

2. By signing a contract, you are expected to fulfill your agreement.  Contract penalty fees do apply.  Upon submitting your 30 day cancellation notice, you will also be charged a $250.00 cancellation fee for opting out of your membership contract agreement before the expiration.  Your final monthly payment and cancellation fee will be charged upon receipt of this cancellation form. 

3. As stated in your membership agreement, contracts will automatically renew upon expiration.  30 day cancellation notice is still required to terminate contracts at the end of their lifespan.

4. If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.
